Cadillac Repair: Tension Belt, power steering pump, alighnment

I'm having a problem with belt it keeps coming off and fraying. Can you give me an idea of what is causing this problem?

Without looking at the car, I can only give suggestions of potential issues. If nothing has been replaced like the water pump, alternator, a/c compressor, power steering pump, air pump because these could be issues, because or length and alignment, there is something that could be giving you trouble.

That would be the tensioner itself, moving in and out throwing off belt alighnment. If you are using the correct belt, I would consider changing the tensioner, because it makes the most sense to the problem.
