Answer
Hi Bryan,

Well you are not getting gas, or not enough gas, obviously.  But why not???

When you first start the truck, do you put it on your foot on the pedal at all?  Try putting you foot in various places to see if it helps.  If so, you could have a dead spot on the TPS.

Also, turn the key on, then off, then on again before you try to start the truck.  This pressureizes the system.  If this works, could indicate a lack of enough pressure at start up.

You may have a vacuum leak, that is causing too lean of air/fuel mix at start-up.  Once started, the IAC motor takes over the idle.  Once it is warmed up, the O2 sensor corrects for any discrepencies.

Does the injector spray anything at cold start?  Or is it dry?  Maybe a pressure regulator.

See what happens with these ideas.

Good luck.
