Answer
Hi Jason,
I prepared an answer to your question this morning but then I accidentally hit the "reject the question" button. If the answer was not there in full when you got the  "rejected" answer, let me know and I will re-compose it and send it again. I believe we can solve this one!

Hi Jason,
You didn't mention which engine so I have to be generic. I would verify that all the items driven by the belt are not bound up due to a damaged bearing. Then I would take a straight edge and place it across each of the pulleys to make sure that all of the devices and any tensioner pulley are all in the same plane with oneanother. If the belt coming off happens in the rain then I would suspect that a splash shield between the tire and the front of the engine is missing (usually part of the inner fender or nearby to that, which intercepts water coming from where the right front tire meets the ground). If the tensioner is a spring-loaded arm make sure that it isn't wobbling on its pivot axis.
Those are the typical causes of belt coming off. Also, check the tension on the belt, it could be too loose. How much does it deflect at a point half-way between the longest separation between pulleys?
If you want to tell me more, use the "thank/rate" tab to get back without having to wait for me to be 'available'.
