Chrysler Repair: 2002 caravan se, throttle position sensor, charcoal canister

Hi Kevin, I have a 2002 4 cyl. caravan se with 105k miles that is stuttering and stalling at takeout. Idles fine and starts fine. It does seem to over rev when the air is on. I am getting codes 1684,0123,0121,0442,0455. It wants to die as soon as my foot touches the accelerator. Also, I notice that at highway speeds, it tends to feel uneven. I cleaned the throttle system some time back and added some injector cleaner but it didn't do much. It will run normal for a short time then starts again.The evap system seems very complex so I am not sure where to start. Any ideas? thanks, Pete

P0121-TPS VOLTAGE DOES NOT AGREE WITH MAP
P0123-THROTTLE POSITION SENSOR VOLTAGE HIGH

as for the evap leaks  u will haft to trace down the harness and see where some cracks are   
look under the van near the charcoal canister  and under the air filter housing.
