Dodge Repair: 2000 Dodge Grand Caravan transy problems, dodge grand caravan, tranny lines

Question
Our 2000 Dodge Grand Caravan started slipping into neutral  when going from reverse to drive, and also slips into neutral while coasting, sitting at stop signs, lights, etc.  While driving, it does not shift smoothly from gear to gear and is getting worse.  We get regular oil changes and have been replenishing transmission fluid.  The check engine light rarely comes on, and this does not seem to be worse at certain times of the day or in cold weather.  Ideas?  My husband does not mind working on the car, as it saves $, but does this sound like something that is fixable by us or only by a mechanic?  Anything you can suggest would be great (including info on costs and how to repair if possible).  THanks.
