Answer
It's been a long time since I've had one of these doors apart, so I don't remember where the fasteners are.  My answer will be based on both my recollection and the information in the shop manual.  Since I'm not certain of how many fasteners there are and where they're located, you should check around the edges of the door panel and under any trim plugs to be sure you haven't missed any screws that will keep you from removing the door panel.

If you don't have power windows, you'll need to remove the crank handle.  To do this, rotate the crank so it's pointing up.  Insert a screwdriver down into the space between the crank handle and the trim washer and pry back on the washer while turning the crank.  This will prevent the washer and retaining clip from turning with the crank and pop the clip off.

Next, remove the screws attaching the trim panel.  There should be one at the top and bottom of the door grip handle.  According to the shop manual, there's also one in the duct of the defroster vent.  Again, be sure to check to verify you haven't missed any.

The door trim panel should now pull outward off of the door.  Pry loose at the bottom and continue pulling outward on the bottom to pull the panel loose all the way up to the sill.  You'll notice that the panel will hang up on the bezel for the door handle.  My efforts to remove this bezel by prying with a screwdriver or similar instrument tend to do a poor job removing the bezel and a great job of damaging it.  I've found that when you reach this point, best results occur by snapping the door panel outward (again, from the bottom) to pop the bezel off.  Once you have the door trim loose all the way up, lift it off of the sill.

If you'd like, you can also pry off the tringular mirror mount cover to make the final removal easier.  If you do this, you should probably do it prior to attempting to pull out on the door panel.
