fan switch only works on high speed . how do I get swich off to replace.does it pull off?

It is not the switch.  There is a resistor cluster in a small plastic enclosure that is screwed onto the air box under the passenger side of the dash.  Go down to the parts store and ask for the resistor box that controls the fan speed for your year Extera and when you look under the passenger side of the dash you will see it.  With the ignition off replace the unit and you should be fine.
