Answer
Ok, while I am in Alabama and not totally familiar with CHP rules, paperwork and such I do have an understanding of them..

The CHP form 180 which has been adopted by many cities also, is a police form which is written up and contains vehicle information about the vehicle and the reason it was towed,  AT POLICE REQUEST..

Your question says it was towed from assigned parking which I take it as from like an apartment complex or such..  Since it was not on a highway, street or roadway (based on answer provided) then there would not be a Form 180 used to authorize the tow...
Now there might be a different form which is tow company specific dealing with the authorization to impound the vehicle from a parking space.  This usually has the signature of the person authorizing the tow...

Now if that is what you are asking for you might have to go the court route to get it...

One thing to remember is that POLICE tows are different than PRIVATE PROPERTY tows in paperwork and such, and will have different rules..

Now if you are talking about an abandoned vehicle report, that is used to notify the local public safety agencies of vehicles towed that would be a different form .

I think the whole thing is based on who ordered the tow of the vehicle here and from the sounds of it you were asking for something that does not apply in this case.   That is what I am seeing in the question asked here.
