Toyota Repair: 93 previa dies and sputters, egr system, egr valve

I've seen a couple other questions like this, but was curious on your take for my previa LE.  Again, it's a 1993, has 202,000 miles on it (automatic, all trac, not supercharged).  It will start up fine, even if it's cold, and run great for a couple minutes.  After 10 minutes or so of actual driving (not just running in the parking lot) every time I stop at a light or stop sign it will want to die.  I've taken to driving with two feet to keep a little bit of gas when slowing down so it doesn't die.  If it does die, it will start right back up, but when I put it is gear it tries to die again.  I should also mention that it is very loud, and I've been told there is a hole in the exhaust manifold, but it's had the hole for several months and this problem is only a couple weeks old and is getting worse every time I drive.  By getting worse I mean the time from start up to when the problem starts is getting smaller.  Any help is appreciated.  Thanks!

The only thing I can think of at this time providing that the engine is tuned up and has no mechanical problems is that the EGR system may be the cause, if the egr valve is staying open even a little bit at idle it will cause idle problems and this condition will not turn the check engine light on.
