When they replaced the throttle body did they also replace the idle speed control valve because that is what the real problem may be.
I am sorry they did replace the idle air control valve/motor and cleaned the throttle body ! Any other ideas ?  Thanks Jim

Answer
I think I have an idea now what the problem may be:
Since the idle is not rough until put in gear(from what you have told me) I suspect there is a motor mount problem, especially if you feel a vibration in the steering wheel(is this the case?) If so, have the front motor mount near the radiator checked, also check the link mount on the front of the engine for any cracks on the rubber insulators, if there are cracks replace both of them.
