Volvo Repair: 1996 850, clutch fork, slave cylinder

we have changed the clutch assy and had flywheel machined, customer has come back 2 months later with a slave cylinder over extended locking the pin out replaced the slave and bleed system ok for two weeks then towed in with same problem

the only time I have seen similar problem was a bent clutch fork.actually I believe the one I had was actually cracked where it attaches to the ball stud on the transmission.it was over extending the slave cylinder.this was awhile ago and only saw this once so this is just a guess.
