Question
Do the Japanese spec S600 cars differ mechanically from the European and American versions?

What should i look (out) for when buying one of these cars? I live in Queensland Australia and there is a Japanese import for sale

Am I crazy to even think about it?

I am not aware of any variation for Japan.
Australia, Brazil, European Union, Russia and USA are the prime variations.

Regarding Japan, I am only aware of the (costly) difficult yearly inspection, graduated vehicle age TAX = every year it costs more to own the vehicle.

My advice is to ask your local Mercedes dealer to check the vehicle identification number, any variation should be noted by this.
