Click on "Forms and Manuals" and "Request for Mail Order Forms."
Type your name, address, telephone number and email address.
Scroll down to "Personalized and Special License Plates Application" (Form # DOR-1716). This form is also used to order disabled license plates. Type a "1" in the box to indicate that you are requesting one form.

Download a Physician's Statement for Disabled License Plates/Placard (Form # DOR-1776).
Take the physician's statement to your doctor and have him complete the form, detailing your disability and certifying it by signing it.
Complete the special license plate application. Remember to indicate that you are applying for permanently disabled license plates.
Mail the completed special license plate application, the physician's statement and all required fees to Motor Vehicle Bureau, P.O. Box 100, Jefferson City, MO 65105.
Receive your permanently disabled license plates and use.
