In the demand letter, you simply describe it and place a dollar figure next to it.  You'll need to attach a letter from your doctor explaining the residual injury, an estimate of future medical bills, and an opinion regarding limitations on work in the future.  You'll also need to attach proof of your income before the accident, which could be tax returns or a statement from your employer.  Sometimes people include a letter from an accountant estimating future values, but this is only if we're talking about a lot of money.  Good luck.
