Answer
Hi Joe,
I wouldn't know how far off the crank has to be compared to the cam shafts to cause hard starting but it is a good observation to keep in mind.
Assuming that you have no other codes and that you have spark and fuel pressure the only other possibility that I can suggest is that the mixture if off, too lean, because the egr valve is stuck open ajar which leans out the mixture too much to sustain combustion. Look for that valve in the pipe that runs from the front side exhaust manifold over to the throttle body intake area. It's body is bolted into the pipe and has a round vacuum actuator on top with a flange in between the valve body and the top. Look inside the flange where you will see a rod with a circumferential slot which is the valve stem. You take the tip of a screwdriver and insert it in the slot so that you can move the stem back and forth, against spring action in one direction that is trying to close the valve. This stem can get cruddy where it enters the valve body which gums up the closing action. So take some WD-40 and spray it on the stem where it enters the body to loosen up the action and get the valve to close securely. Then try and start the engine and see if that corrects the no start.
