Toyota Repair: Cam timing after head milled, crank pulley, dowel pin

I have an 89 Toyo Corolla with 4AF engine.  Replace head gasket.  Machine shop milled maximum allowable metal off of head. (asked for min) Am certain that cams are installed correctly, cam correctly lined up with crank.  Engine won't start.  Will spit and try if bolt out of distributor and dist. advanced all the way to bumping on engine.  Tried changing cam timing with timing belt, (moved a tooth) and engine will spit and sputter if dist. retarded all the way to bumping on engine.  Heard there was a spacer I may need to install under the head.  Any ideas?

Resurfacing the cylinder head has absolutely no effect on the timing belt timing and no spacer is required, I've never even heard of this.
There is still a cam or distributor timing issue.
Make sure the bottom crank pulley mark is lined up with the zero mark on the timing cover and that the #1 piston is all the way up then make sure the cam gear is lined up correctly, sometimes they used the same cam gear for two different engines, make sure you use the 4AF-E mark on the gear with the dowel pin of the camshaft pointing straight up, remove the distributor cap and locate where the #1 piston plug wire contact is on the inside of the cap, make a paint mark on the distributor and be sure that the rotor points exactly to the mark.
