In Arizona, the Department of Transportation’s Driver Services division oversees the awarding of licenses, vehicle registration and management of vehicle records. You can obtain a copy of your Arizona motor vehicle record online, through the mail or in-person.
To obtain your MVR online, visit the online portal from the ADOT Driver Services website. Choose the type of record you want, either driver history MVR or title and registration MVR. Enter your full name, driver’s license number, date of birth and the last four digits of your Social Security number. Provide your payment information, confirm the transaction and print out your record. As of 2015, each record costs $3. You cannot request another person’s record online and can only get uncertified records via this method.
From the ADOT Driver Services website, print out and fill in the Motor Vehicle Record Request. You’ll need the same information you need for the online form—full name, driver’s license number, date of birth and last four digits of your Social Security number. Include $3 for each uncertified record and $5 for each certified record you request. To find the nearest motor vehicle department, use the search tool on the ADOT Driver Services website.
When requesting another person’s MVR, you must indicate your reason for wanting it by choosing one of several permissible uses set by the Federal Driver’s Privacy Protection Act. All the permissible uses are listed on the Motor Vehicle Record Request form. You must have a MVD agent or another notary witness and notarize the Motor Vehicle Record Request form.
