How to Obtain a Disabled Parking Permit in Utah

If you are a disabled resident of Utah, you may qualify for disability parking permits. Your disability condition must meet one or more of the disability criteria required under the Utah Division of Motor Vehicles (DMV) rules. Qualifying conditions include heart conditions, circulatory or lung disease, a diagnosis affecting the lower extremities, loss of extremities or vision problems.

  • Visit the 4dmv.com website and review the Utah DMV listing of disability conditions which are necessary for qualification as a disability.

  • Download the Disabled Person and Physician Disability Certification form TC-842 from the Utah Tax Commission DMV website.

  • Enter the information requested in Section C which pertains to the applicant. You should choose the type of permit that you need, either disabled person license plates or the windshield placard.

  • Ask your physician to complete the Physician Disability Certification for Individual Applicants in Section A. Your physician indicates whether the disability is temporary or permanent, and how long the temporary disability will last.

  • Send the completed application and physician certification to the Utah State Tax Commission DMV at the address on the form. You also can take the form to your local DMV office. There is no fee for placards, but the disability plates have a fee in addition to your regular registration fees.
