If you have a disability, whether temporary or permanent, you can apply for a disability parking permit in Michigan. Determine if you meet the state of Michigan's definition of temporary or permanent disability which restricts your mobility, and that it can be certified as such by a physician. The qualifying conditions are described on the Michigan Secretary of State website.
Download and print either the application for a disability placard or a disability license plate from the Michigan Secretary of State website. Disability placards are issued for temporary or permanent disability. Disability plates are only available for a permanent disability. Complete Part One on the front of the application form.
Take the application to your physician or other recognized health professional, so that Part Two of the form can be completed. The physician will only complete Part Three of the placard application form if you meet the conditions to be entitled to free parking.
Mail the completed application to the Michigan Department of State at the address on the form. You can also take the application to any Michigan Secretary of State branch office. After approval, you will receive a blue placard if you have a permanent disability and a red placard if it is a temporary disability.
