How to File for a Lost Title for an Auto in Indiana

Records for vehicles registered in the state of Indiana are maintained by the Indiana Bureau of Motor Vehicles (BMV). A vehicle title is an important document which legally establishes the owner of the vehicle. Titles are required to prove ownership, as well as to officially transfer ownership of a vehicle. If you have lost your title, apply for a duplicate copy through the Indiana Bureau of Motor Vehicles.

  • Duplicate Title Affidavit Form 22620
  • Driver's license or photo ID
  • Check or money order
  • Find the nearest branch of the Bureau of Motor Vehicles by visiting the BMV website. Search by city or county. Take note of the operating hours of the selected location, as some branches are closed on Monday.

  • Complete a Duplicate Title Affidavit Form 22620. Provide the year, make and vehicle identification number (VIN) of the vehicle. Check the box next to "Lost" to indicate the reason for the request. Forms are available at the local branch or online at the BMV website.

  • Acquire the notarized signature of the lienholder if a lien exists on the vehicle. This is required in order to release the title to anyone other than the lienholder.

  • Sign and date the form. Return the completed affidavit to the local branch in person or by mail. Provide a copy of your driver's license or government-issued photo ID. Include payment for the duplicate title in the form of check or money order payable to the Indiana Bureau of Motor Vehicles. As of 2010, the cost for a duplicate title is $9.

  • Ensure that the address on file with the BMV is current, as the duplicate title will be sent to you by mail (usually within four to six weeks). Address changes may be submitted online or at the local office.
