Auto Insurance Claims: authorization form, medical authorization form, medical providers

Do I need to sign a medical authorization form?  What would happen if I don't?  Thank you.

Hi Tina,

If you are presenting a bodily injury claim you must sign the medical authorization. This form is to allow the insurance comapny to contact all of your medical providers to obtain the medical records and the medical bills. If you do not sign the form they will deny your claim. The only exception to this is if you are represented by an attorney. Then you should only do what your attorney tells you to do.
