Audio Systems: Directional speaker, range speakers, ear level


Question
I would like suggestions for building a directional speaker.
Or a general design for a box to install the speaker in that would be directional. The application is directing audio (from a television) towards an individual that has hearing loss so that person would get the volume they desire but the sound would be minimized in the opposite direction so unimpaired individuals will not have to endure high sound volume. I have placed multiple home theatre speakers in a box and directed the box towards the individual. I still get high volumes from the rear of the box. The speaker box will be about 2 feet from the hearing impaired individual and about 1 foot below his ear level

I am sure they want full range sound and that they do not want to listen to just super-tweeters so this going to be about impossible since the lower the frequencies are, the wider the speakers' dispersion becomes.  It is not impossible, but it is not cost effective to engineer such a system when the best resolve for this is a pair of comfy headphones.
I understand that if you get them close enough to the listener, a set of speakers (at low volume) can act like headphones and do the things which you desire but it simply is not cost effective. You can buy closed back full range speakers, but they are just emulating a set of headphones.
