Chrysler Repair: Belt Tensioner Assembly misalignment -98 Plymouth Voyager 3.3L V6 FI, plymouth voyager, haynes manual

My belt tensioner makes a funny noise every now and then. With the engine running, it looks like the belt is not alligned with the other pully's. In other words the tensioner seems to be on a angle. I removed the belt and the tensioner seems tight, but when I put the belt on and run the engine it misalligns again. I tried to tighten the mounting bolt but can't get to the nut. I thik I need to replace it, but my haynes manual does not tell me what parts I need to take off to access the bolt. Can you help?

The surpentine belt on the 3.3 is driving all those separate units so the tensioner is given quite a workout.
While I haven't personally worked on it, as I look at the various manuals I have it appears to me that you might want to remove the splash shield that separates the right front wheel well from the engine compartment. My '93 van manual suggests that the shield be removed even when you are just trying to change the belt, after first lifting the right front of the van from the ground (hoist or jack). That may well give you full access to the tensioner mechanism. The other part you might want to look at is the bracket that holds the power steering-generator-tensioner and that has several attachments to the engine and the exhaust manifold. Perhaps one of its bolts is loose which might cause the tensioner to be out of the plane of the other pulleys.
That is my assessment based purely on looking at the manuals. I would appreciate your letting me know if this is helpful and in any case how the repair works out.
