Chrysler Repair: engine coolant system, chrysler concorde, coolant system

Hello,
  I have a 95 chrysler concorde and a few days ago I was driving and the temp gauge went all the way to hot so I put water and coolant in it and now I can drive it maybe 5 mins before the temp goes up, I have not let it go past 3/4 but I am not sure why this is happening? Any suggestion? I had the water pump and timimg chain replaced in feb of 2005.

this could be a bad thermostiac or a pluged rad first i would have the thermostiac changed and if that does not work then have the rad. checked to see if pluged and also could be a head gasket on the way out . good luck to u and if you need more help please let me know and please rate my serivice pete
