Ford Repair: Ford Windstar Rocker Arms, ford windstar, oil hole

Hi, I have repaired a Windstar for a friend twice in the last month for the same problem. She has a 96 windstar 3.8L.
Two rocker arms have "broken". The Pushrod has protruded through the oil hole in the cup on the rocker arm. First was on #1 intake. Fixed it, worked for a week. Next was #5 exhaust. Both times was the same problem. Now it has happened again, I think. I haven't been able to take apart yet to check, but sounds and acts the same. Is there something that makes this happen, or is it just because they're old? I'm going to replace the rest this time if that's the case, I just wanted to make sure something else isn't causing this problem.

I am a Honda technician, and I can't find any info anywhere on this failure.

i usually only see this with oiling problems on the fords is it sludged any if so might have to try and clean it out something i don't like doing because you know what the consequences could be. if not sludged i am at a loss at this minute but i will think on it.
