Ford Repair: spindle nut, spindle nut, inner pad

I'm replacing the calipers and rotors on my '93 Ford ranger and need some guidance on how to tighten the spindle nut during installation......
Also, how is the inner pad held in place?  The one (loaded caliper) I bought came with the inner pad loose while the outer pad is clipped to the caliper.
