Answer
Hi.  Technical service bulletin (TSB) 01-40-006-1 is especially pertinent to your issue.  If you do not have an account at www.hmawervice.com, I'd recommend registering for a free account there so you can view the bulletin.

Basically, the bulletin is a process-of-elimination series of repairs.  Based on what's been done so far and the recommendations in the bulletin, I'd recommend next replacing the reed switch in the speedometer head.  This switch converts the rotational motion of the speedometer cable into a pulse signal received by the engine and transmission control modules.

If the problem returns after replacing the reed switch, the cause is somewhere in the wiring between the TCM and pulse generators.  The recommendation in the TSB is to replace the harness at this point, but if you're at this point and it's more economically viable, you could try running new wiring between the pulse generators and the TCM.  Keep in mind, however, that if you do not replace the terminal ends as well, you haven't eliminated problems with the terminal ends as a possible cause.
