Nissan Repair: noise when in gear, cam follower, nissan pickup

i own a 95 nissan pickup the truck runs great starts every time but when you are sitting in gear with foot on brake it has a loud ticking sound this only seems to happen when in drive not in nutral or rev or park, a friend said might be the flywheel but would appreciate your opinon

Sounds like the flywheel to me.  The vehicle needs to be put on a lift and have someone start it, put it in gear with the brake on and follow the tick.  If it ticked all the time under load I would say the tappets (cam follower dampners) but since it is only doing it in drive and stopped I would think flywheel but would need to hear it to confirm.
