Nissan Repair: studs and lug nuts keep stripping, lug nuts, lugs

Question
what would cause a rim and tire to fall off when lugnuts were tight? i already put
new studs and lugnuts in the rear because i only had 4 holding the back tire on,
on each side. a couple of weeks after i did that i took the Pathfinder to the shop
to get tires mounted. after that that's when the tire passed me at 45 mph.i didnt
roll the truck or anything, it went surprisingly straight.

That happened to my dad when he was young. Glad it never happened to me! Lucky it was a rear one! I would guess that the lugs nuts weren't tight or the lugs were inferior in quality. If the rim is not a factory rim, it might not have a good fit and that also would cause this to happen.
