Pontiac Repair: 98 Grand Prix AC HEATER Question, vacuum line, blower motor

About six months ago the switch on output stopped working.  I can not change it from defrost to floor, etc.  What part is needed to fix this problem.

Second Question.  The blower works on all 5 settings, but makes an aweful noise.  Do I need to just replace the blower motor?

Hi, on the defrost im not sure what part is needed to fix this, because it could be a vacuum line that has came loose, or a door is stuck and will not open, this could be the switch is not working. IF motor is making a noise in all setting then just replace the motor.
