Toyota Repair: Rough idel, 22re engine, vacuum lines

My 1986 4 runner, 4 cyclinder,auto trans, 230K, 22RE engine has a rough idel.  I replaced all of the small vacuum lines and it does run a little better but not great.  The truck also has one other symptom.  When the engine is hot and I turn it off for about 20 minutes and then restart it, it runs very rough and almost stalls.  I rev the engine for about 45 seconds to make it run smoother.  Any idea of the cause?

Are you sure is an RE (fuel injected) and not a carb?
If it is a carb engine the carb needs to be cleaned or rebuilt.
If its fuel injection then the rubber intake should be taken off and
inspected for cracks. If its ok then the alum intake must be cleaned
out with a whole can of carb cleaner. With the rubber intake off spray it in there with your face turned away, its toxic.
Then jack up the other side of vehicle so a lot of the residue runs out. lower and remove the jack and start the vehicle, it will start hard but will start and blow clouds of smoke out of the exhaust.
After it starts idling smooth, take it for a test drive.
