Toyota Repair: Auto problem, toyota caldina, shift points

I have a 1994 Toyota Caldina that intermittently takes off in second gear from a standing start i.e. does not select first.  Most of the time the auto drives and works well. This fault can occur when the auto is cold or warm and either soon after the car is started or having been driven for a while.  The OD light also flashes intermittently when the fault occurs.  When he fault is evident first can be selected via the gear lever it just will not select 'automatically'.

I'm not familiar with a Toyota Caldina. How many miles? Is it a 3 or 4 speed auto?
First thing to do is change the transmission fluid and filter.
If its a 3 spd the shift points are controlled by the governor and
throttle body. If its a 4 spd the transmission is controlled electronically with shift solenoids and they have signals sent by
the computer(ECM).
