As for Toyota Service Bulletins, I did a search on the internet and found three that specifically address vibration in the brakes for the Previa:
BR00797 - APR 97 - New front pad reduces vibration
BR95002 - JULY 95- Changes Reduce Vibration / Pulsation
BR94002 - FEB 94 - Cause and Repair of Vibration and Pulsation
Unfortunately, I do not have access to these documents.  I would be eager to know what they say - especially BR94002 and BR95002, which seem to relate directly to the very issue I'm dealing with.

Answer
BR007-97 bulletin only applies to Previa models 1994-1996, since yours is a 1991 it does not apply.
The TSB BR95-002 dated July21,1995 does apply, it is a 4 page document and if you can give me an email adress I can forward it to you.
BR94002-94 only adresses the advantages of using an on board brake lathe versus a normal brake lathe, this bulletin is outdated and offers no additional information.
