Heel and toe wear is where the leading edge of a tread element is rounded due to the way it enters the tire footprint (heel), and the trailing edge is pointed, also due to the way it leaves the foorptint (toe).  It resembles a foot - hence the name.

To some extent, heel and toe wear is the way tires normally wear.  However, excessive amounts of heel and toe wear, have a variety of causes - alignment, excessive torque, excessive braking.  This, of course, is aggravated by insufficient inflation pressure and insufficient rotation practices.
