Used Cars: Title Issues, lien holder, true title


Question
I have a brother who had my grandma help him with buying a car.  My grandma is the primary on the loan and my brother is the co-signor.  My grandma died this year.  My brother wants to trade in the car but my Grandma's name and his name is on the title.  The title says my Grandma's name AND my brothers name.  Can my brother trade the car in without having it go through probate first.  The debt is in my Grandma's name.  I say No because the lien holder views the Primary as the true title holder he said that because there is an AND between their two names he can sign over the car as part of the trade in.  This is in KY.
