Download and print the appropriate form which is applicable to the type of disability permit that you are selecting and qualify to receive. If your disability is temporary, download form MVR-37T to request a temporary placard. If your disability is permanent, download form MVR-37A for the permanent disability placard or form MVR-37 for the disability plates. Complete the required applicant information on these forms.
Give the application form to your physician for certification that your disability meets the North Carolina requirements for impaired mobility.
