Requesting the title in person

  • Go to your local West Virginia branch of the DMV. Request the Affidavit of Duplicate Title for a Vehicle or Watercraft.

  • Fill out the Affidavit of Duplicate Title for a Vehicle or Watercraft completely, taking time to answer each question as fully as you can. You may need the VIN number or other identifying codes for the title search.

  • Present the completed form to the clerk along with the required fee, which was $10 as of 2010. The clerk will provide the title history and if you requested it, a duplicate title.

Requesting your title online

  • Go to the West Virginia website and navigate to the Vehicle License Information section. Download the Affidavit of Duplicate Title for a Vehicle or Watercraft.

  • Fill the form out online and then hit "Submit." You will be directed to the secure payment page where you must pay the fee with a valid credit or debit card. Hit "Submit."
