How to Obtain Your MVR in Colorado

The Colorado Division of Motor Vehicles maintains Motor Vehicle Records, or MVRs, for drivers in the state. These records contain the last seven years of a driver's activity. You can order either a certified or non-certified copy of your MVR from the DMV, both of which cost a fee.

Order In Person or By Mail

  • If you want to order an uncertified copy of your MVR, you can do so at any of the state's driver's license offices. The fee is $2.20 as of 2015. However, to receive a certified copy, you must visit the DMV in person at 1881 Pierce St., Lakewood, CO 80214 or mail your request to P.O. Box 173350, Denver, CO 80217-3350. Certified copies cost $2.70 as of 2015. You can also request uncertified copies by mail. Send your request by either USPS First Class or Priority mail. The DMV does not accept Federal Express, UPS or other delivery services.

Personal Information Required

  • When requesting your MVR, you must provide your first and last name, birth date, photo identification, and driver's license number if you have one. You must provide this same information when submitting your request in writing, except you'll provide a copy of your ID. Make sure your signature is fully visible on the photocopy. You must also provide an original signature on your request, such as at the bottom of your letter.

Acceptable Forms of Payment

  • If requesting your MVR in person, you can pay with cash, check, Visa, MasterCard, American Express or Discover card. For requests by mail, don't send cash. Send either a check or money order payable to the Department of Revenue. The DMV will send your MVR 10 to 14 business days after receiving the request.

Submit Release to Order Another Person's MVR

  • If you are requesting someone else's MVR, you must complete a "Requester Release and Affidavit of Intended Use" form. Mark the box that best describes why you are entitled to the record, such as if you have the other person's written consent. You can deliver the release form to the DMV either in person at its 1881 Pierce Street location or by mail to P.O. Box 173350, Denver, CO 80217-3350. The release form is available online from the Colorado Department of Revenue's website.
