Auto body repair & detailing: door hinges, door hinges, body style

Question
can you please tell me how to get too the one bolt on the door hinges on a 1978 gmc 1/2 ton.the repair manual does'nt help!

I just sold my truck this body style so I cant go look,But I have a feeling that the fender will have to be loosened or removed.There is a possibility that the rear bolts can be removed and then spin the hinge to access the last bolt.It will be a slow process but it beats taking off the fender.Also you can replace just the pins and bushings if a saggy door is the problem.
