Chrysler Repair: 2002 Dodge Caravan Radiator Fan Relay, radiator fan, dodge caravan

Hi Kevin,
Hope you can help...my radiator fan just keeps running, no matter what it won't shut off....do u know where the relay fan is located and how it looks so I can locate it...also what is the cause and how can this problem be fixed?  Please help I am desperate.

REMOVAL
Open hood.
Disconnect and isolate the battery negative cable.
Remove the radiator crossmember to front fascia closure panel.
Disconnect the relay electrical connector (Radiator Fan Relay).
Remove the rivet attaching the relay to the front bumper beam (Radiator Fan Relay).
Remove the relay.
