Honda Repair: 88 Accord Lx-i hard starting, gas milage, tune ups

Question
2 issues,
1- the car idles smoothly in park and neutral, as soon as it goes into gear the car/dash/steering wheel vibrate. Motor mounts have been replaced already
2-recently only after sitting overnight or long enough to completely cool, sometimes very hard to start. Takes turning the motor over for roughly a minute before it will start, sometimes will catch, stumble and die immediately. Once it starts it runs as normal. Not sure where to start looking.

Answer
Tony, you have some issues going on. First the timing should be checked. But, before you do that when was the last time you did a tune-up? Assuming your car has high miles, 100K +, tune-ups should be performed every 30K. Tune-up = spark plugs, spark plug wires, distributor cap, distributor rotor, and a fuel filter. Try these then have the timing checked again. Your car is missing in time or misfiring. Whichever performing these basics will not only help you pinpoint where to go next for a diagnosis but give you better performance, gas milage and reliability. If this still doesn't fix the problem i would suspect the fuel pump or the ECU.
