Plymouth Repair: Vibration after replacing front brake pads, pep boys, auto zone

Question
On a 1998 Plymouth Silhouette - On applying the brakes at above 40 M/Hour there was a vibration in the steering wheel. At high speed or going down hill the vibration was worse, almost dangerous. Last month a mechanic said it was a warped rotor and charged me for turning both sides.(1000 miles ago.) The vibration was completely gone. Since I had used cheap pads from Auto Zone and was hearing a squealing when pressing on the breaks, I decided to change over to ceramic pads from Pep Boys. I replaced the pads myself. I did not bleed the lines. I pried between the caliper housings and rotor to squeeze fluid back into the reservoir.  When done, the vibration upon pressing the brake, is back. This time it is really bad. I also noticed that the lug holes on the rotors do not align well around the lugs as the holes are off center and are about a 1/16 of an inch larger than the lugs. The center hole is nicely centered on the axle hub but the lug holes are not. Is this my reason for a vibrating rotor or is there a certain way (trick) to put the wheel/rotor back after changing pads? I did use a star pattern when tightening lug nut.

Sorry for being long winded and thanks in advance for any help.

Sincerely
Vibrating at the wheel.
