Answer -
DG,

A MILLION MILES? WOW!

What colour is the smoke?

White = Coolant
Blue  = Oil
Black = Carbon (diesel)

White is the most common, if the vehicle is starting and running good.  It is often caused by
a bad headgasket or a leak at the oil cooler.  Look for some sludge buildup in the coolant
reservoir.

Good Luck,

Paul

Answer
DG,

OK, that sounds better.  Black smoke in fifth gear, may be a sign of a plugged intake manifold or that your turbocharger is worn.  You'll need to have it checked out, because it will only get worse. A mechanic would remove the intake boots to check for excessive sludge build up, and check for excessive impeller shaft play. Repairing it now will save fuel and reduce the damage to the engine. This condition referred to as coking.

Good Luck,

Paul
