If you know your run flat did not operate with low pressure why can't you patch it from inside. What makes this different than patching a regular tire.

When repairing conventional tires, there are signs that the tire has been damaged by operating without sufficient inflation pressure (commonly called "run flat").  In RunFlat tires, those signs are not present.  That's why tire manufacturers do NOT recommend repairing RunFlat tires - you can't tell if the tire is damaged.
