Trucking: Lease Agreement, lease agreement form, sole proprietorships

Question
how can i get a lease agreement form for a small mushrooming trucking company. Here is the situation, tractors are owned by individuals or sole proprietorships, trailers are mostly rental with possibility of tractor and trailer owned in a similar manner. Some of the owners are not the drivers, we(company) are to hire the drivers at a 50/50 share of the performance after OTR expenses(Dispatch charge, fuel, tolls, lumper); repairs are to be serviced by the tractor owner.
