How to Get a Disabled License Plate in Arizona

If you are in Arizona, permanently disabled and are driving with standard plates that do not allow you easy access to your most frequented haunts, consider applying for a disabled license plate. Disabled license plates guarantee reserved parking spots and can simplify your travels.

  • Logon to Arizona's Department of Transportation website (see Resources below). Click on "Motor Vehicle Division."

  • Select "Forms and Publications" and "MVD Forms."

  • Type "96-0104" in the form number box and press "Search."

  • Verify that a link directing you to a disability license plate application shows.

  • Click on the disability license plate application, download and print it.

  • Check "New Plate."

  • Print your name, mailing address, date of birth and driver's license number.

  • Provide your vehicle's identification number, year, make and current plate number.

  • Read over the descriptions of the disabilities that make an applicant eligible for disability license plates. Confirm that you have one of disabilities detailed.

  • Take your application to a physician, audiologist or official hospital administrator. Have the agent indicate whether you are permanently or temporarily disabled or if you have a hearing impairment. Request that the agent provide the hospital's name and mailing address, its phone and fax numbers, his name, medical license or certification number and the current date.

  • Sign and date your application.

  • Total your fees. Make your check payable to Motor Vehicle Division.

  • Mail your completed application and check to Mail Drop 801Z, Special Plates Unit, Motor Vehicle Division, PO Box 2100, Phoenix, AZ 85001-2100.

  • Receive your disability license plates. Display them only on the car legally registered under your name.
