Hi Dipak,
The P1491 code refers to the radiator fan control circuit. In the later '90's model years the most common reason for a problem of this sort is the solid-state fam control module which is located on the chassis frame rail on the left side of the engine compartment, just behind the radiator in your '97. It failed so regularly that Chrysler did a recall so if your has not already been replaced you may be able to get it done for free at a dealer. It is a quick replacement in any case that you can do yourself. You remove the air intake box by the battery and then you will see it down below mounted on the frame rail.
The P1768 is about the transmission control relay not providing power to the solenoid pack on the side of the transmission. As the result the trans goes into limp-in mode where it will only operate in one forward gear, 2nd gear. If that is what you have experienced, try replacing the relay which is located in the box under the hood near battery where it is the 3rd relay counting from the rear of the box. If it isn't the relay, then the wires involved in the circuit would need to be checked for a possible open or shorted condition.
