Plymouth Repair: 1991 grand voyager transmission problem, toronto ontario canada, little old lady

Question
My wife just purcased a 1991 grand voyager from a little old lady that claimed it had only been driven to the cottage every weekend. 138,000 km on it and it is mint on the inside!. We purchased it three days ago in Toronto , Ontario Canada. Because we only drove it around the block we didn't get above 60 km. My wife came home today and said something was wrong, so i took it on the highway and realized that it was not shifting out of second gear?. It roared like it was going to blow up! Tranny fluid is perfectly clear. Mechanic did safety on it only one week ago. Paid $1700.00.Also it grinds ( growls ) in front end , yet brakes seem fine. Like a grinding sound? My wife is freaking because I told her about this deal through a co-worker. The engine looks very clean. I know the mechanic that has looked after this truck. Should he have done safety certificate knowing this issue? Hope you can offer some advise. When we called little old lady she seemed genuine. Friends that know her say that the truck was babied.
