If you know where the vacuum leak is, the best way to fix it is to replace the hose or tube that has the leak.  If it's a loose or broken connection that's causing the leak, some hi-temp gasket sealant can be a temporary fix.  

There is a different procedure to to test the MAF in every car.  Find a service manual for your car (most auto parts stores sell these) and it will give the diagnostic procedure.
