You must meet the qualifying conditions under Pennsylvania law and obtain a certificate from an authorized health care provider, or in some cases, such as blindness, from a police officer. Make sure that you meet the disabled eligibility requirements under Pennsylvania Bureau of Motor Vehicle rules. There are many qualifying conditions listed on the PennDOT website including blindness, limited mobility, use of portable oxygen, cardiac conditions and arthritis.
Download and print either Form MV-145A Person With Disability Parking Placard Application or Form MV-145 Application for Person With a Disability or Hearing Impaired Registration Plate. Complete the applicant information in Section A on the upper part of these forms.
Take the form to your physician or other authorized health care provider to obtain certification of your disability and to classify it as temporary or permanent. In certain cases, such as blindness or if the disabled person does not have full use of one or both legs, a police officer can certify the disability.
Mail the completed application to the Pennsylvania Bureau of Motor Vehicles at the address shown on the application form and include the application fees, if any, as indicated on the form. The plates and placards are sent to your home address.
